Windward Life Care’s interdisciplinary team of Aging Life Care Managers have certification and professional training in a number of areas related to healthy aging, including nursing, geriatric care management, and social work. Clients facing various health and mental health challenges, including Alzheimer’s, other dementias, and Parkinson’s disease, greatly benefit from our combined staff’s expertise and integrated model of care. In addition, our home care aides undergo training in these areas and work under the close supervision of the Aging Life Care Managers to follow each client’s individualized plan of care.
Purpose
Windward Life Care’s specialty is providing the expert in-home care and home health care our client’s need, wherever they are. And by “wherever they are,” we mean both where they physically live and where they are on their aging journey. Through our integrated model of care, clients benefit from the combined skills of everyone on the Windward team. This includes expertise in social work, nursing, dementia care, and other related fields.

Windward Life Care was founded in 2004 to provide premier home-based care services to San Diego’s older and disabled adults. Drawing on many years of experience in geriatric care management, in-home care, and home health, Windward offers clients the highest degree of professional service and hands-on care available in San Diego.
