Thresholds

Established in 1959, Thresholds provides healthcare, housing, and hope for thousands of persons with mental illnesses in Illinois each year. Through care, employment, advocacy, and housing, Thresholds assists and inspires people with mental illnesses to reclaim their lives.

Thresholds is one of the oldest and largest providers of recovery services for persons with mental illnesses in Illinois. We reject the notion that anyone is a lost cause, utilizing evidence-based practices and a wide range of supports to treat the whole person, rather than just the disease.

We offer 25 innovative programs at more than 70 locations throughout Chicago, the adjacent suburbs, and McHenry and Kankakee Counties. Services include assertive outreach, case management, housing, employment, education, psychiatry, primary care, substance abuse treatment, and research. Last year, Thresholds served more than 7,000 adults and youth, with 75% of services delivered out in the community, representing more than 400,000 hours of care.

Purpose

We offer 30 innovative programs at more than 75 locations throughout Chicago, the adjacent suburbs, and four surrounding counties. Services include assertive outreach, case management, housing, employment, education, psychiatry, primary care, substance use treatment, and research.
Thresholds’ staff meets people where they are – out in the community. Offering services in a community-based setting makes them more accessible and effective and lessens the social exclusion that is often present in the lives of persons living with mental illnesses and substance use conditions. This allows our staff and clients to work on skills, clinical interventions, and recovery that allows them to live independent lives in their own neighborhoods
Our clients are all unique – coming from vastly different backgrounds and arriving with different needs, talents, and hopes. Each client works with our staff to set their own specific short-term goals and objectives, based on their needs, desires, and level of self-sufficiency. Thresholds rejects the notion that anyone is a lost cause, and we utilize evidence-based practices and thoughtful supports to treat the whole person, not just their mental illness. Our foremost goal is to ensure that every individual in our care, called members, receives the necessary skills and support to live an independent and fulfilling life.

Thresholds’ staff meets people where there are – out in the community. Instead of providing services in a clinic-based setting, 80% of Thresholds’ services are delivered out in the places where our members live and work. This allows our staff and members to work on skills, clinical interventions, and recovery that allows members to live independent lives in their own neighborhoods. Offering services in a community-based setting makes them more accessible and effective, and lessens the social exclusion that is often present in the lives of persons living with mental illnesses.

Thresholds’ recovery model is to work with members to create their individual path to recovery, rather than force at-risk individuals into a rigid set of one-size-fits-all programs. Our members are all unique—they come from vastly different backgrounds and arrive with different needs, talents, and hopes. Each member works with Thresholds’ staff to set their own specific short-term goals and objectives, based on their needs, desires, and level of self-sufficiency. We then connect each member to the programs and services that will help them reach their specific goals.
