Provides primary medical care for people of all ages, including chronic disease management (asthma, diabetes, HIV), mental health outpatient counseling and treatment, supportive social services including health education and parenting skills classes, Women, Infants and Children (WIC) nutrition program, lead poisoning prevention, dental, physical therapy and care in schools. Eligibility and enrollment assistance for BadgerCare, Affordable Care Act marketplace and other community support programs. All services can be provided in English, Spanish or Hmong. All services are available to people regardless of insurance status, with a discounted fee scale based on family size and income for individuals who are uninsured and below 200% of the Federal Poverty Level.
