We coordinate services that help people to live independently in their homes and communities. We use a person-centered approach to determine individual needs and connect our participants to services that enhance their independence
and keep them in their homes and communities. These services include home care, nursing, meals, transportation, community participation, and home modifications. SCU provides service coordination and supports coordination services to individuals with physical disabilities, the aging, individuals with intellectual disabilities, as well as to their families.

Purpose
We assist our participants in developing an individualized plan to provide in-home care keeping them comfortable in their own homes and participating in their community.
