Founded by volunteers in 1990, Rainbow Community Care is a local, independent, and non-profit provider of services that supports patients and families throughout the entire journey with aging and serious illness.
Highly regarded for excellence in hospice care, palliative care, advance care planning, grief support, parish nursing, and volunteer programs, we care for people living in their homes, assisted living facilities, and nursing homes in rural South-Central Wisconsin as well as the Rainbow Hospice Care Inpatient Center in Johnson Creek.

Purpose
We are building a community to serve a community when and where care is needed most.

We are building a team and establishing relationships with partners who share our dedication to serving the community and providing care where and when it is most needed.
Over time, Rainbow Community Care – in collaboration with local clinics, hospitals, nursing homes, and assisted living facilities – has transformed Jefferson County into "an island of excellence" for care delivered in the later stages of life. For example, Jefferson County is now a place where more people choose to receive hospice care in their final days, weeks, and months than anywhere else in Wisconsin.
Achievements like this do not happen overnight nor do they happen by accident. They only occur in places where an organization is dedicated to helping people live well as they age and become seriously ill, and knows how to work with others to achieve that goal. And, they only occur in places where an organization's leadership understands that it's the skill, knowledge, and values of its staff that leads to the creation of something special.

Guiding our day-to-day work at Rainbow is a set of values centered around caring for the most vulnerable people living among us. Much like firefighters responding to an alarm, we choose to run toward something most people want to avoid.
