Physician Health Partners (PHP), with locations in Denver and Colorado Springs, focuses on improving the health care delivery for more than 500,000 Coloradans. PHP is a vibrant and innovative company whose goal is to improve the health outcomes and experience for their patients, while managing the associated costs.
PHP was founded in 1996 by primary care physicians. PHP works with two clients, PHPprime and Colorado Community Health Alliance (CCHA). PHPprime is a group of more than 300 primary care providers in the Denver and Colorado Springs metro areas and PHP helps them manage Medicare Advantage and commercial contracts. CCHA is the Colorado Medicaid entity for Boulder, Broomfield, Clear Creek, El Paso, Gilpin, Jefferson, Park and Teller counties.
