PACE Southeast Michigan
PACE Southeast Michigan is a unique health plan, providing home and community-based care to help nursing-home eligible adults aged 55 years or older with chronic illnesses remain independent, in their own homes. Our expert care team works with our participants to develop a personalized care plan, including social, medical, and other PACE services. We provide access to a full continuum of preventive, primary, acute, and long-term care services 24/7, 365 days a year in our day health centers across Southeast Michigan and in the home. By aging in place, older adults have more hope, vitality, joy, and an overall higher quality of life. PACE (Program of All-Inclusive Care for the Elderly) is a national program funded by Medicare and Medicaid. Founded in 1994, PACE Southeast Michigan is a non-profit organization with eight centers located in Southfield (including the Corporate office), Dearborn, Eastpointe, Sterling Heights, Clinton Township, Pontiac, and two centers in Detroit.
Purpose
To support nursing-home-eligible older adults with chronic, complex health needs living in their own homes through the end of their lives.

By 2025, as a top performing PACE program, we will keep over 3,000 older adults with chronic conditions independent!
