McLean Home Care and Hospice is a Medicare certified vising nursing agency specializing in post hospital skilled care, orthopedic rehabilitation, geriatrics and hospice care. Our multi-disciplinary team includes Registered Nurses, Physical therapists, Occupational Therapists, Speech Therapists, Medical Social Workers, and Home Health Aides. Our services include Joint Replacement Rehabilitation, Fall Prevention, Stroke Rehabilitation, Wound Care, IV Management, Breath Easy COPD Program, Living with Heart Failure Program, Tele monitoring, Lifeline Emergency Response System, Dementia Care, Advanced Planning and Resources, Palliative Care, and Hospice Program.
