Families and Community Together (FACT)
Purpose
FACT (Families and Community Together) is part of NJ's Children's System of Care and is the designated Care Management Organization for Union County. We exist to help the youth of Union County to succeed at home, in school, and in their communities. We work with young people (from ages 5 and up until their 21st birthday) and their families with emotional, behavioral, developmental, mental health, and substance use needs. We practice a wrap-around approach, a family-oriented and community-based model of care that addresses barriers to well-being, maximizes the youths' and families' strengths and helps the family identify and build a team of supports.
Vision
That all the youth and families we serve are able to live successful, healthy, and independent lives in their homes, schools, and community.
Mission
FACT partners with youth, families, and their communities to develop plans that will help the youth achieve stability, positive change, and success at home, school, and in their community.
