Connecticut Community Care, Inc. (CCCI) identifies choices and provides services to support people of all ages, abilities and incomes to live at home. We respect the dignity, rights and contributions of all individuals and we believe that self-determination is critical for older adults and people with disabilities. Over the past 35 years, CCCI has developed a “best practice” model of care coordination. Care managers conduct comprehensive assessments; develop person-centered care plans that address strengths, preferences and unmet needs; make arrangements for in-home services and monitor and modify plans as needed. CCCI serves more than 12,000 individuals each year through the CT Home Care (CHCP) Program. Hundreds of others receive services through Money Follows the Person (MFP), hospital care transitions, private care management and chronic disease self-management programs. Coordinated community-based services are a dignified, efficient and effective alternative to institutional care. Each year, CCCI saves Connecticut millions of dollars.
