Compassionate Care Hospice is a licensed, certified provider of end-of-life care throughout the state of Delaware. We provide disease management, symptom management, and family support for seriously ill patients and their families. Our interdisciplinary team consists of physicians, nurses, nurse’s aides, social workers, chaplains, volunteers, nutritionist, wound care nurse, and music therapist. We care for patients wherever they live, at home or in a facility. Our model of care is holistic; we focus not just on physical symptoms, but also on psychological, spiritual, and social/family concerns as well.
