The Vita Health & Wellness Partnership is a COC-led coalition of community service agencies initiated to promote public health and wellbeing. This vital network includes Stamford Hospital, Optimus Healthcare, New Covenant Center, Family Centers, Children’s Learning Centers and many other local partners all working together to develop innovative community-based programs designed to impact the social determinants of health (www.VitaStamford.com).
