At Center for Hospice Care, “care” is at the heart of everything we do. Founded in 1978 as a community-based nonprofit, we have served over 47,000 patients, providing hospice, home health, grief counseling, and community education.
We believe everyone deserves to live on their own terms, especially at the end of life. Hospice care empowers patients and families to make meaningful choices. Unlike corporate providers, our nonprofit status ensures a commitment to compassionate, community-driven care.
Serving nine northern Indiana counties, we operate offices in Mishawaka, Elkhart, Plymouth, and La Porte, and operate the region's only two Medicare-certified inpatient facilities: Esther’s House and Ernestine M. Raclin House.
Since caring for our first patient in 1980, we have kept our promise that no one will be turned away due to financial circumstances. At Center for Hospice Care, people come first --- because every person deserves dignity and support when it matters most.

Purpose
Center for Hospice Care (CHC) is a mission-driven organization dedicated to improving the quality of living through compassionate care for those facing life-limiting illnesses. As a community-based, non-profit leader in hospice and palliative care, CHC provides expert medical, emotional, and spiritual support to patients and their families, wherever they call home. With a legacy of service spanning decades and a commitment to honoring every moment, CHC not only ensures comfort and dignity at the end of life but also offers comprehensive grief and bereavement support, community education, and international partnerships that extend its impact far beyond our home base in northern Indiana.
