Care Dimensions is the nonprofit leader in serious illness care, dedicated to providing compassionate, comprehensive services to patients and families across Massachusetts. Since its founding in 1978, the organization has upheld a mission centered on dignity, comfort, and personalized support during some of life’s most challenging moments. With a full continuum of services—including hospice, palliative care, HomeMD primary care at home, dementia GUIDE program, grief support, and two inpatient hospice houses—Care Dimensions delivers expert, patient‑centered care to thousands of individuals each year.
