APEX Health Services is an Illinois-based healthcare organization, founded in 2008, dedicated to improving the quality of life for individuals and families through compassionate, patient-centered care delivered wherever patients call home.
Our interdisciplinary team provides coordinated care across Home Health, Hospice, Palliative Care, Transitional Care Management, Chronic Care Management, Home Care, Private Duty, and Medical Group. By working collaboratively across service lines, we create seamless transitions, improve patient outcomes, and ensure every individual receives the right care at the right time.
While our services continue to grow and evolve, one thing has remained unchanged since APEX was founded more than 18 years ago: our people. The same values that inspired our founders to put patients first continue to guide our employees today. Because exceptional healthcare isn't defined by the services an organization offers—it's defined by the people who deliver them.
