Halcyon Hospice and Palliative Care is a non-profit agency committed to a standard of excellence in providing sensitive, patient centered, health care to hospice patients and families.
In addition to providing unmatched hospice care, we have also created a palliative care program that not only supports patients as they prepare for end of life transitions, but it is also designed to anticipate, prevent and treat symptoms holistically and to facilitate transitions across care settings (to hospital, SNF, etc), care providers and goals of care.
