Landmark Home Health Care Services
Landmark Home Health Care Services provides skilled nursing, physical and occupational therapy, speech language pathology, social services and home health aides. Landmark Home Health is a non-profit, Medicare certified agency, dedicated to helping patients achieve the highest possible quality of life, while building independence, through a prompt response to patient needs. Landmark offers a Telehealth program. By taking an active role in managing heart failure and other health issues, it helps to prevent hospitalizations and improves the overall health of the patient. All services are provided in accordance with a physician signed plan of care. Landmark has also become a provider for Personal Emergency Response Systems (PERS). The small, lightweight units are available to our patients which helps in the event of an emergency at home or away from home. Another division of our agency is Landmark @ Home, they have been providing services for 22 years. They are a provider for Veteran Affairs, Human Service Department Grant Programs, Options, private duty and home health aides. They provide assistance where it is needed, shopping, meal preparation, light housekeeping or personal care.
