Neighborhood Family Practice is a network of community health centers that delivers high quality, affordable primary and preventive health care to people in Cleveland's diverse west side neighborhoods. We provide comprehensive, personalized care and the support and education to foster better quality of living for patients, their families and communities.
NFP provides primary care, women’s health, behavioral health, dental care and pharmacy services for patients of all ages. Our six locations offer day and evening hours, same day appointments and 24/7 access to a health care provider. Services are accessible to all income levels as NFP accepts Medicaid, Medicare, most commercial insurance plans and offers a financial assistance program (sliding fee scale) for the uninsured.
NFP is accredited by the Joint Commission and is recognized by NCQA as a Patient Centered Medical Home (PCMH Level 3 - highest).
